It definitely looks like a custom piece. The gold also looks very yellow, so it might 22k, which would narrow its origins a little. Here is something I found about the clasp: http://www.google.com/patents/US4697315

I know very little about these things but it looks Italian to me! They often use lots of coloured stones and yellow gold...
